Description of Service: Case management and direct instruction, ChildFind screenings and evaluations, early intervention and special education programming in physical gross/fine motor skills, cognitive skills, speech and language skills, social/emotion skills, and adaptive skills.
Location: Penobscot County
Geographic Area: Androscoggin, Aroostook, Cumberland, Franklin, Hancock, Kennebec, Knox, Lincoln, Oxford, Penobscot, Piscataquis, Sagadahoc, Somerset, Waldo, Washington, York counties
Intake Procedure: Contact the CDS Case Manager at 947-8493 or by fax at 990-4819 for more information; case manager will contact family after receiving the Child Find intake referral.
Intake Requirements: Child's name and date of birth, parents' or guardians' names, address, and phone number; area of concern.

Eligibility: Children from birth through 5 years of age in need of early intervention or special education programs.
